Preparing for Your Appointment
To ensure a timely and efficient visit, we kindly ask that you arrive 15 minutes before your scheduled appointment. This allows time for completing registration materials, updating your insurance records, and paying any required co-pays. Please come prepared with the names, dosages, and frequency of all medications you are currently taking. We recommend writing this information down in advance. Don’t forget to include any non-prescription medications, such as laxatives, herbal supplements, and vitamins, to give your provider a complete view of your health.
Insurance
We accept most major health insurance plans and health maintenance organization (HMO) programs. If you are unsure about whether your insurance is accepted, please call our office, and we will be happy to confirm. As a courtesy to our patients, we file all insurance claims.
However, it’s important to note that since all insurance contracts are between the patient and the insurance company, the responsibility for payment rests with you. We strongly recommend reviewing the following aspects of your health insurance policy to avoid any surprises:

Referrals
Whether you need specialty care, emergency treatment, hospitalization, or diagnostic testing depends on your specific condition. Unless it's an emergency, we prefer you contact our office first before seeking care outside of our practice. This allows us to coordinate your care effectively and refer you to the appropriate specialist if needed. Please give us a minimum of two working days to facilitate the process.
Prescriptions
For prescription refills, please reach out directly to your pharmacist and request the refill. We recommend contacting your pharmacist at least 48 hours in advance and during our office hours. Be sure to provide the prescription number and relevant information from the label. Ask the pharmacist to fax the refill request to our office.
Please note that prescriptions for antibiotics, pain medications, or other non-routine drugs require an office visit. This allows us to review your medical history and conduct an examination to ensure that the prescribed medication is appropriate for your treatment plan.
