For New Patient Appointments:

Before your first appointment, please download the forms below, print them, and complete them. Bring the filled-out forms to your first visit. Also, be sure to complete the Phreesia pre-registration sent via text or email. This will be sent to you a few days before your appointment.

Your Initial Visit:

Please arrive 30 minutes before your appointment to give us time to review your information and create your chart. Bring a complete list of your medications and your insurance card(s). If you have any relevant medical records, please bring those with you as well.
Kindly be advised that personal checks are not accepted for payment during your first visit. We do accept cash, credit cards (VISA, MasterCard, and Discover), and money orders.
